Initial Comments:
Based on the findings of an off-site follow up survey completed on January 26, 2022 , Excela Home Health Care & Hospice was found to have corrected the deficiencies cited under 42 CFR, Part 418, Subparts A, C, and D, Conditions of Participation: Hospice Care. The deficiencies were cited as a result of an on-site state relicensure and Medicare Recertification survey completed on September 27, 2021.
